Continuing Care

YEARLONG CARE FOR PERMANENT SUCCESS

Wellspring Wisconsin campers participate in Continuing Care program to smooth the transition from camp to home.  This online Continuing Care program ensures that campers stay on the path to a healthy weight over the next year.  Campers have access from the end of camp until June 1, 2011.

The Continuing Care program requires campers to continue self-monitoring and goal-setting, as well as journaling, planning activity and weighing-in weekly. Their Behavioral Coach from camp – i.e., someone with whom they’ve established a strong relationship – tracks their progress and continues to provide feedback and support.

During the last week of Wellspring Wisconsin, BCs introduce campers to the Continuing Care program.

Wellspring believes so strongly in the Continuing Care program that campers who successfully complete the program will be refunded the entire cost of the Continuing Care program ($600).

The fee is refunded based on meeting two criteria:

1. Camper completes 140 out of 180 days of self-monitoring (3 food items with fat and calories recorded per day)
   
   
2. Parent (or camper) obtains a verified height and weight and provides this form back to Wellspring via mail, fax, or email.
